Interlibrary Loan Form (For current students, faculty, and staff only)
If you are trying to borrow from a library in the OPALL consortium, you may initiate a request from an item in the OPALL catalog by clicking on "request" and inputting your barcode and last name. The book will be sent to our library to be picked up by you. If there is a book or article that we DO NOT HAVE in our library collection or in the OPALL Consortium, fill out this form and library staff will borrow it for you from another library. Please make sure that you check the library catalog and Ebsco Host before filling out this form.
